Sometimes, what gets lost in an instant takes a lot longer to get back. Perhaps no one in south Orange County knows that better than Dr Boris Del Cid.

After breaking his back seven years ago, Dr Del Cid has partnered with his former SCI rehab trainer, the man who helped him regain a measure of his independence, to open a chiropractic care center in San Juan Capistrano.

Apex Chiropractic & SCI Wellness Center specializes in treating the kind of spinal chord injuries that Dr Del Cid himself suffered. Technology has allowed him to practice in ways he couldn’t have imagined during those six months he spent in the hospital wanting to find a way to get back to the life he’d known.

Dr Del Cid has lived in San Clemente with his wife Jill for the last 14 years. He came to the United States from Panama in 1980, at age 18, to study law. After discovering chiropractic, he switched from law and built a successful career in the field.

But on Valentine’s Day 2006, his life changed.

“That was my second birth,” Dr Del Cid says.

Dr Del Cid was riding his motorcycle near the Tustin Marketplace. After a traffic light turned green, he made a left turn, while a car at the opposite side of the intersection turned right—heading the same direction as Del Cid. The driver then changed lanes, into his path.

“I hit her in the rear bumper and catapulted over the car,” Dr Del Cid said. “I broke my back at the T3 and T4 vertebrae. I should have died. In fact, I did expire at the hospital for about 16 seconds.”

The chiropractor is paralyzed from his chest down and he still has a titanium rod in his femur.

Dr Del Cid appreciates the irony—the chiropractor with a broken back.

“I ponder that all the time. It’s comical, laughable. All I can think about is what’s going through their (his patient’s) mind,” Dr Del Cid said.

He said he realized the severity of the injury right after the accident, lying in the street, when he couldn’t feel his legs at all.

“I do remember, vividly, making my peace with God. I said, ‘If you want to take me, I’m ready to go now. I want to go, because I don’t want to live like this.’ I was there, literally, waiting to die. That happened within the first couple of days of being there,” he said.

After contracting pneumonia at the hospital, Dr Del Cid suffered respiratory failure and needed to be intubated to get air into his lungs.

“The experience of dying was terrifying, the actual physical feeling of not having air. After that, I woke up. From there on, I really didn’t have a fear of dying. I was just thankful to be alive. I didn’t die because of a reason. I’m very spiritual, so I’m going to say that God didn’t want me to die.”

While the driver’s insurance did not cover his hospitalization, Del Cid said the community, his friends and neighbors in San Clemente, rallied around he and his wife. They raised over $50,000 at a fundraiser, along with helping his family try to regain some normalcy.

“I owe so many people,” Dr Del Cid said.

Jill Del Cid said she didn’t comprehend the severity of her husband’s injuries and the accident until she arrived at the hospital.

“I was at work when I got the call,” she said. “I thought I’d just have to pick up Boris and see about his bike. When I got to the hospital, they immediately let me know that he would never walk again.”

Initially, she said, she planned on working through these changes herself.

“I didn’t think I needed any help,” she said. “But people would come to the hospital, the neighbors held a barbecue for him, they did the fundraiser. They would walk our pets and prepare meals for me.”

Jill said she had to work much harder to make up for the lost income. She’d encouraged her husband to find work, but knew he wanted to return to chiropractic care.

“Everything worked out perfect,” she said.

Dr Del Cid said he understood the sacrifices his wife has made since his accident, especially after a neighbor had told them to prepare for potential bankruptcy due to the loss of income.

“She’s a hero, she’s everything,” he said.

After leaving the hospital, Dr Del Cid said he was unsure what he was going to be able to do. He still had the desire to be a chiropractor.

“All the other skills, diagnostic skills, observation, anything that requires your brain to do, I can still do,” he said.

About a month after he left the hospital, Dr Del Cid’s motorcycle mechanic gave him a call. The mechanic was a former patient and an amateur boxer.

“When he heard I’d gotten out of the hospital he called me up. He asked me how I was doing. He asked me ‘could I come visit you? I want you to check my neck.’ I was going ‘What?’” said Dr Del Cid.

The mechanic had boxed the night before and thought his neck may have been out of alignment. “I was like, ‘Dude, I’m a month out of the hospital. I can’t walk.’”

The mechanic was undeterred. Before he arrived, Dr Del Cid said he worked out a way to use the limited mobility he had to do what he could while seated, adjusting the neck.

“Exactly how I was going to do it, I didn’t know. But I told myself I was going to try,” he remembered.

When his friend arrived, Dr Del Cid said the skills he’d learned with his hands had clearly not left him. He was able to make the adjustment. When he realized he could still do that work, he said, it was a major relief. But there was still self-doubt to overcome.

“I was just coming out of the hospital, I thought, ‘This is ridiculous. Who’s going to want to get adjusted by me? I can’t even move.”

But the precedent had been set, he said.

And with the encouragement of his wife and friends, he set off on the goal of returning to work. While he did rehabilitation at the Project Walk rehabilitation center in Carlsbad, he continued to perform adjustments on friends and neighbors and kept up his chiropractic license by attending continuing education classes.

Dr Jeremy Sontag was the rehab trainer who worked with Dr Del Cid at Project Walk. The two became good friends as they worked to give Dr Del Cid back a measure of independence. One day, when Dr Sontag complained about back pain, Dr Del Cid recommended a colleague in Encinitas. After that visit, Sontag told Dr Del Cid he wanted to become a chiropractor himself. Dr Del Cid made him a promise.

“I told him, when you’re done with chiropractic school, if you want to practice, call me up and let’s see if we can get something going,” Dr Del Cid said. However, Dr Del Cid explained that he didn’t necessarily hold Dr Sontag to that. Since schooling can take up to four years, Dr Del Cid said he figured Dr Sontag would likely prefer to work with a fellow student or someone else.

But Dr Sontag said he never had any desire to go into business with anyone else. The two communicated regularly while Dr Sontag was in school, and when graduation approached, the two began to talk seriously about going into business together.

“He’s always had a great outlook,” Dr Sontag said. “He’s always had a passion for helping people.”

While some of his classmates might have been able to do more physically, Dr Sontag said Dr Del Cid’s experience helping to build a practice and goodwill in the community was something that wasn’t taught in school.

“He always had the intention of getting back into practice, it was just a matter of how he was going to do that,” Sontag said.

On the advice of a fellow chiropractor, Dr Del Cid discovered a special instrument that allows him to adjust regions of the spine that wouldn’t otherwise be able to reach. The machine uses sound waves to find out if an area is out of alignment, and then can be used to perform the adjustment as well. The adjustment table is itself on a hydraulic lift that allows Dr Del Cid to put his wheelchair underneath it.

In addition to his new business, which is celebrating a grand opening Saturday, Dr Del Cid is active in helping others who’ve suffered similar injuries. He’s partnered with others on a social networking site for those who’ve suffered spinal chord injuries, www.pushrim.com, and does a regular podcast that discusses living with spinal chord injuries.

Plainly described, it is the misalignment of a vertebra in the Spinal Column as it stacks above and below another.

The Subluxated vertebra is connected to the above and below vertebrae by discs and ligaments. These three vertebrae are what we call a Vertebral Motor Unit, due to the ranges of motion that can be achieved by and with them.

But what is the problem with a misaligned or subluxated vertebra? THE NERVE(S)!!!! It impinge them.

The NERVES that exit the Spinal Column, do so from the sides between two vertebrae. The name of the area where that nerve(s) exit is called a FORAMEN.

When a vertebrae is misaligned, it alters the FORAMEN because the vertebrae are mobile. As a result, the nerve exiting through the foramen, can become compressed, impinged, irritated, and inflamed.

This can cause restricted range of motion, muscle stiffness, and pain. However, the real questions we should ask are: Where do these nerve go? What structures or organs do they supply or activate?

EVERY ORGAN and EVERY CELL in the human body WORK because of the NERVOUS SYSTEM. Decrease or interrupt the nerve supply to any of these structures and what do you get?
LESS THAN full function of these structures. What is the cumulative effect of this? Adaptation!!! to less than optimal function of organs that, form systems, that work with other systems, that make the body work in homeostasis or the BALANCED EQUILIBRIUM OF INTERDEPENDENT SYSTEMS.

That is the REAL PROBLEM with SUBLUXATION.

In the advent of computerized work, instant information, social media and texting, we’ve become exposed to physical stresses that can adversely affect our health.  Unbeknown to us, involuntary biomechanical changes occur in our bodies when engrossed in our technology devices upon extended sitting and even while standing.

 A consistent group of signs and symptoms have been identified in relation to these stresses.  It is called Corporate Syndrome (CS).   In simple terms is a pattern of muscular imbalances caused by repetitive or prolonged use of computers, tablets or cellular phones.  Furthermore, CS can present itself in the upper body and lower body.  The specific classifications are: Upper Crossed Syndrome (UCS) and Lower Crossed Syndrome (LCS).

 Signs include for UCS, rounded shoulders, forward carrying of the head, neck and trapezius muscle tension and stiffness.  Symptoms may include, neck pain, headaches, wrist pain, tingling sensations to the hands and fingers, rotator cuff problems and accelerated arthritic changes of the cervical spine.

  For LCS, signs can be comprised of tightness of hamstrings, hip flexors, and Ilio-tibial band, in addition to, weakness of the abdominals, gluteus maximus/medius, oblique’s, and vastus medialis /lateralis.  Accompanied by symptoms such as, low back pain/stiffness, sciatic neuritis (sciatica), disc problems, and accelerated arthritic changes of the lumbar spine.

In conclusion, an imbalance of Tight vs. Weak opposing muscle groups, thus the term “Crossed”, can cause postural problems that unbeknown to us may result in neuro-musculo-skeletal conditions.  The sooner these conditions are identified the better, regardless of how young or old you are.  Remember the lack of “pain” does not mean that everything is “O.K.” In fact some of the most insidious diseases and conditions, never give you a “pain” warning.

Plantar Fasciitis is a condition characterized by pain and inflammation of a thick band of tissue called the plantar fascia.  This band of tissue runs across the bottom of the foot connecting the heel bone to the toes.   This plantar fascia acts like a shock absorber supporting the arch in the foot, much like the tension in a bowstring.  When this tension becomes too much, small little tears appear in the plantar fascia where it attaches to the heel.  This Repetitive stretching causes irritation and inflammation.  The pain experienced is often stabbing usually feeling worse upon getting out of bed in the morning.  As the plantar fascia and surrounding structures get more pliable and warmed up, the pain can diminish.  Later in the day during periods of prolonged standing or getting up from a seated position, the pain can reoccur.

This condition is common in runners, ballet dancers, overweight individuals and those who routinely wear shoes with poor support, like high heels.  It is most common with people between the ages of 40 and 60.  Having a high arch, being flatfooted or having faulty foot biomechanics can affect how bodyweight and tension are placed on the plantar fascia. 

When plantar fasciitis is left untreated it can lead to chronic heel pain, inflammation, and injuries to other weight bearing joints of the body.  Among them are, the knees, hips, ankles and low back.  Also it can promote unsafe walking patterns which can lead to joint dysfunction and muscle imbalances such as adaptive shortening of the calf and leg muscles. 

How can Chiropractic help?

Chiropractic care addresses the cause of plantar fasciitis.  Usually, patients suffering from plantar fasciitis present at least one of the mentioned causes.  If the exacerbating activity is not stopped, the plantar fascia will have little chance to heal.  Though stopping all activities is not necessary, some level of modification will be required. This is where chiropractic treatment can help.

Foot Adjustments- Chiropractic adjustments to the affected part of the extremity are usually indicated. The bones in the foot can be adjusted to improve biomechanics between the layers of muscle and fascia.

Has anyone noticed that in the last few years, the word Wellness, has been surfacing everywhere?  In fact, many insurance companies include wellness  in their advertising and coverage plans.  Indeed, such a concept has been the center of a shifting paradigm in the healthcare business.

The sciatic nerve is the longest and widest nerve in the body, running from the lower back through the buttocks and down the back of each leg. It controls the muscles of the lower leg and provides sensation to the thighs, legs, and the soles of the feet.

Although sciatica is a relatively common form of low-back and leg pain, the true meaning of the term is often misunderstood. Sciatica is actually a set of symptoms—not a diagnosis for what is irritating the nerve root and causing the pain.

Sciatica occurs most frequently in people between the ages of 30 and 50 years old. Most often, it tends to develop as a result of general wear and tear on the structures of the lower spine, not as a result of injury.

What are the symptoms of sciatica?
The most common symptom associated with sciatica is pain that radiates along the path of the sciatic nerve, from the lower back and down one leg; however, symptoms can vary widely depending on where the sciatic nerve is affected. Some may experience a mild tingling, a dull ache, or even a burning sensation, typically on one side of the body.

Some patients also report:
• Pins-and-needles sensation, most often in the toes or foot
• Numbness or muscle weakness in the affected leg or foot

Pain from sciatica often begins slowly, gradually intensifying over time. In addition, the pain can worsen after prolonged sitting, sneezing, coughing, bending, or other sudden movements.

How is sciatica diagnosed?
Your doctor of chiropractic will begin by taking a complete patient history. You’ll be asked to describe your pain and to explain when the pain began, and what activities lessen or intensify the pain. Forming a diagnosis will also require a physical and neurological exam, in which the doctor will pay special attention to your spine and legs. You may be asked to perform some basic activities that will test your sensory and muscle strength, as well as your reflexes. For example, you may be asked to lie on an examination table and lift your legs straight in the air, one at a time.

In some cases, your doctor of chiropractic may recommend diagnostic imaging, such as x-ray, MRI, or CT scan. Diagnostic imaging may be used to rule out a more serious condition, such as a tumor or infection, and can be used when patients with severe symptoms fail to respond to six to eight weeks of conservative treatment.

What are my treatment options?

<![CDATA[FIBROMYALGIA]]> https://hdalbis.com/blog/b_41356_fibromyalgia.html https://hdalbis.com/blog/b_41356_fibromyalgia.html The word  fibromyalgia comes from the Latin term for fibrous tissue (fibro) and the Greek ones for muscle (myo) and pain (algia). Fibromyalgia syndrome is chronic disorder which includes widespread muscle pain, fatigue, and multiple tender points that affects 3-6 million people in the United States. For reasons that are unclear, more than 90% of those who develop fibromyalgia are women. It is not currently known whether the predominance of women who suffer from fibromyalgia is a phenomenon of the socialization of women in the American culture or whether it is some combination of the female reproductive hormones and other genetic predispositions.

According to the American College of Rheumatology (ACR), fibromyalgia is defined as a history of pain in all four quadrants of the body lasting more than 3 months. Pain in all four quadrants means that you have pain in both your right and left sides, as well as above and below the waist. The ACR also described 18 characteristic tender points on the body that are associated with fibromyalgia. In order to be diagnosed with fibromyalgia, a person must have 11 or more tender points. In addition to pain and fatigue, people who have fibromyalgia may experience:

  • sleep disturbances
  • morning stiffness
  • headaches
  • irritable bowel syndrome
  • painful menstrual periods
  • numbness or tingling of the extremities
  • restless legs syndrome
  • temperature sensitivity
  • cognitive and memory problems (sometimes referred to as "fibro fog")

Fibromyalgia is often confused with another condition called "myofascial pain syndrome" or "myofascitis." Both fibromyalgia and myofascitis can cause pain in all four quadrants of the body and tend to have similar tender point locations, but the two conditions are worlds apart. Myofascitis is an inflammatory condition due to overuse or injury to your muscles, whereas fibromyalgia is caused by a stress-induced change in metabolism and healing. Myofascitis tend to come on rather suddenly and is usually associated with a particular activity or injury, true fibromyalgia has a slow, insidious onset, usually beginning in early adulthood. It is very important to diagnose each of these correctly, for they require very different approaches to treatment. Unfortunately, fibromyalgia is a chronic condition, meaning it lasts a long time - possibly a lifetime. However. it won't cause damage to your joints, muscles, or internal organs.

The Basics of Fibromyalgia

The latest research indicates that fibromyalgia is a stress-related condition that is a cousin in Systemic Lupus Erythematosis (often referred to as simply 'lupus') and Chronic Fatigue Syndrome. In all three of these conditions, there is the same predominantly female distribution, chronic fatigue, sleep disturbances, irritable bowel, as well as many other similarities. You can think about these three conditions as lying on a continuum with Fibromyalgia on one end, Lupus on the other and Chronic Fatigue Syndrome in the middle. All three of these conditions are caused by an abnormal stress response in the body, but with Lupus, the immune system is primarily affected, causing an autoimmune reaction that attacks your healthy tissues. On the other end of the spectrum is fibromyalgia, where metabolic abnormalities are primary. These metabolic changes are the result of a stress-induced decrease in blood flow to an area of the brain called the pituitary. This, in turn causes a decrease in a number of important hormones, such as the growth hormone releasing hormone (somatotropin) and the thyroid stimulating hormone. These hormonal changes lead to abnormal muscle healing, borderline or full-blown hypothyroid, as well as memory and cognitive changes.

One of the major physical abnormalities that occurs with fibromyalgia lies in the muscle itself, where there is a build up of a protein called "Ground Substance." Ground substance is normally found in muscle, bone and connective tissue all over the body and is responsible for making the tissues stronger and less susceptible to tearing. In a normal person, when a muscle is injured, the muscle tissue itself is able to regenerate and over time, completely heal itself. In a person with fibromyalgia, the muscle is unable to completely heal itself. Instead, an abnormally large amount of ground substance builds up in the injured area. It is the ground substance, coupled with local muscle spasm it creates that creates the muscle 'knots' associated with fibromyalgia.

A number of tests may be done to rule out other disorders and an examination can reveal whether a person has the characteristic tender areas on the back of the neck, shoulders, sternum, lower back, hips, shins, elbows, or knees. Unlike its cousin lupus, there are currently no diagnostic laboratory tests for fibromyalgia. Because there are no clinical tests for fibromyalgia, some doctors, unfortunately, conclude that a patient's pain is not real, or they may tell them that there is little they can do. But a combination of chiropractic, trigger point therapy, and lifestyle changes has proven to be very effective in decreasing the severity and duration of the physical pain and disability of fibromyalgia.

Fibromyalgia can be difficult to treat. Not all doctors are familiar with fibromyalgia and its treatment, so it is important to find a doctor who is. Fibromyalgia treatment often requires a team approach, utilizing chiropractic care, trigger point therapy, massage, dietary changes, as well as exercises and stretching.

Treating Fibromyalgia With Chiropractic

Chiropractic care is critical for those who suffer from fibromyalgia in order to keep the spine and muscles from losing too much movement. Because fibromyalgia causes the muscles to tighten up and lose some of their natural pliability, it results in a global loss of movement in the spine. The loss of movement in the spine results in a neurological reflex that causes the muscles to tighten further. This vicious cycle will continue and over time will lead to increased pain, increased muscle tightness, a loss of movement, more difficulty sleeping and the development of more and more trigger points.

The only option is to continually adjust the spine and keep it moving. It is not uncommon for those with fibromyalgia to be adjusted three to four times per month to keep everything mobile and relaxed. The biggest concern in treating people with fibromyalgia is that their muscles have a diminished healing ability. For this reason, chiropractic adjustments are usually modified slightly to be more gentle than normal. This helps to decrease the stress on all of the small supporting muscles of the spine, which can be easily injured. It is important when seeking chiropractic care, to make sure that the doctor is familiar with the muscular changes that occur with fibromyalgia so that they can adjust their treatment accordingly.

Treating Fibromyalgia with Trigger Point Therapy

The overwhelming characteristic of fibromyalgia is long-standing, body-wide pain with defined tender points, and frequently, trigger points. Trigger points are often confused with "tender points." They are not the same. A trigger point needs firm pressure to elicit pain, while tender points are painful with even very light pressure. Trigger points will refer pain to other areas of the body, whereas tender points will not. Unlike tender points, trigger points can occur in isolation and represent a source of radiating pain, even in the absence of direct pressure. As discussed earlier, trigger points are purely comprised of spasmed muscle fibers, whereas tender points are knots filled with ground substance. Those with fibromyalgia almost always have a combination of the two - trigger points and tender points - and can improve dramatically with light trigger point therapy.

Trigger point therapy for fibromyalgia is much like trigger point therapy for low back pain, neck pain or headaches. The points are the same. The difference is just intensity. Since the muscles in patients with fibromyalgia are easily injured and take longer to heal, it is necessary to use less pressure on their trigger points.

Treating Fibromyalgia with Cold Laser Therapy

Since poor healing of muscle tissue and chronic pain are characteristic traits of fibromyalgia, laser therapy is an important part of any treatment plan. Two of the major benefits of cold laser therapy is stimulation of tissue healing and decreased sensations of pain.

A 1997 study of 846 people with fibromyalgia reported in the Journal of Clinical Laser Medicine and Surgery demonstrated that two-thirds of the patients experienced improved pain and mobility with cold laser therapy. Another study published in Rheumatology International in 2002, showed that those who received laser therapy had a significant improvement in pain, fatigue and morning stiffness.

Self-Care for Fibromyalgia

Your day to day lifestyle choices have a tremendous impact on how much impact fibromyalgia will have on your life. The difference between those who take care of themselves and those who do not is tremendous. Those who make lifestyle changes to help their fibromyalgia suffer much less pain, are able to remain more active and have a much higher quality of life than those who do not. If you have fibromyalgia, here are some of the main things that you can do on a daily basis to help your body:

Getting enough good sleep

-Getting enough sleep and the right kind of sleep can help ease the pain and fatigue of fibromyalgia, but is something that can be hard to get. Many people with fibromyalgia have problems such as pain, restless legs syndrome and brain-wave irregularities that interfere with restful sleep. Insomnia is very common. Although alcohol may help you to relax, it is not recommended before bed as it has been shown to interfere with restful sleep. Some of those with fibromyalgia have found 5-hydroxy tryptophan (5-HTP) very helpful, as well as the prescription anti-depressant amitriptyline. Typically, we don't recommend taking perscritpion drugs, but in this case, it is difficult to heal without enough sleep.

Exercising

- Improved fitness through exercise is recommended. Studies have shown that fibromyalgia symptoms can be relieved by aerobic exercise. Though pain and fatigue may make exercise and daily activities difficult, it's crucial to be as physically active as possible. The best way to begin a fitness program is to start with low impact exercises, like walking and swimming. Starting slowly helps stretch and mobilize tight, sore muscles. High-impact aerobics and weight lifting could cause increased discomfort, so pay attention to your body. The more you can exercise, the better off you will be.

Making changes at work

-Most people with fibromyalgia are able to continue working, but they may have to make big changes to do so. It may be necessary to reduce the number of hours at work, find a job that will allow you to have a flexible schedule, or switch to a less physically demanding job. Many people with fibromyalgia require specially designed office chairs, adjustable desks or other adaptations in order to continue working. If you face obstacles at work, such as an uncomfortable desk chair that leaves your back aching or difficulty lifting heavy boxes or files, your employer may make adaptations that will enable you to keep your job.

Eating well

-Foods, just like anything else, have the ability to either stress your body or to help your body heal. Foods that tend to be stressful on the body include: dairy, eggs, wheat, corn, as well as anything with monosodium glutamate (MSG), nitrates or nitrites (as are found in processed foods). Several environmental toxins may also contribute to the overall physical stress on your body, therefore fish should be avoided as well. It is important that you eat as much clean, organically grown fresh foods as possible. Base your diet around whole foods such as: brown rice, legumes, oats, spelt, rice milk, soy, hormone-free chicken or turkey, roots, nuts and berries.

Nutritional supplements

-There are dozens of nutritional products that claim to be 'the answer' for fibromyalgia. To date, none of them have proven to be of much long-term benefit for anyone. However, there are some people who have used magnesium malate with good results, some people who have used ginkgo biloba with good results and others with various herbals. The bottom line with nutritional supplements is that, to date, there is nothing that works for everyone. If you come across something that you would like to try, by all means do so, as long as you check it out with your chiropractor first to ensure that it won't interfere with any of your other treatment.

The term "pinched nerve" is somewhat of a catch-all phrase that is commonly used to describe the pain associated with a variety of conditions from subluxations, to tunnel syndromes to the referred pain from trigger points.

Most of the time, what is called a pinched nerve is actually an irritated, or inflamed nerve where the nerve itself is not actually pinched. In most cases, nerves become irritated and inflamed when the bones, joints or muscles of the spine are not in their proper position, or are not moving properly. This condition is called a "subluxation", the treatment of which is the specialty of the doctor of chiropractic.

There are instances when nerves do become 'pinched', such as in Carpal Tunnel Syndrome, Sciatica and Thoracic Outlet Syndrome. In each of these cases, injury, spasm or inflammation of the surrounding muscles and connective tissue causes the nerve to become compressed, resulting in pain. These conditions are referred to as "tunnel syndromes." Treating tunnel syndromes is more complex than treating a simple spinal subluxation, but they usually respond very well to chiropractic care; especially when combined with other physical therapies, such as exercises and stretches.

Trigger points are very tight "knots" of muscle that form when muscles are either chronically overworked or injured, and are often experienced as a pinching or burning pain. Trigger points will commonly cause pain that radiates to other parts of the body, which is also known as referred pain. The successful treatment of trigger points usually requires a combination of chiropractic care, stretching and a form of deep tissue massage called 'trigger point therapy.'

Often the main indicator is initial pain in the joint and decreased mobility. Frozen shoulder can affect people of any age from children to adulthood, but is most commonly diagnosed in people ranging from 40 - 70 years of age, predominantly women.

The shoulder joint itself is called a ball and socket joint. Ligaments, tendons and muscles work together to provide support, strength and the wide range of motion that enables us to move our arms and hands in a variety of positions in order to complete tasks. All the functions of the shoulder can be compromised by underlying inflammatory diseases and misuse. The specific causes of frozen shoulder perplex are varied and largely still unknown, but onset begins with initial pain, followed by restriction in mobility and finally recovery.

Frozen shoulder can often be referred to as insidious in nature. The symptoms and development of the disorder are slow and can take up to a year or two to set in. Often patients will experience pain that will increase over time. As chemical changes take place in the shoulder joint, thick strands of tissue called adhesions form and begin to restrict mobility. The lubricating synovial capsule in the shoulder joint thickens and provides less lubrication. By the time the sufferer begins to notice a significant issue in lack of mobility, the disorder has set in and requires treatment. 

The good news is that although the causes of frozen shoulder are varied, treatment is straight forward and the disorder can be resolved. Often clinicians, including our experienced and caring staff, can provide the correct manipulation and physiotherapy to help you regain mobility and resolve the disorder. Causes of Frozen Shoulder
Adhesive capsulitis (Frozen Shoulder) can be attributed to misuse and injury, myocardial infarction, upper torso surgery, such as arm and shoulder surgery or mastectomy, and even lack of use. When the shoulder joint goes unused and remains in the same position for long periods of time, such as when a patient is placed in a sling or unable to use their arm, the joint tightens and mobility is decreased. An autoimmune response in the area may cause the joint to stiffen and restrict the joint causing the initial pain. When the initial pain is not addressed, the inflammatory nature of the disorder will progress and adhesions form in the joint further restricting mobility and increasing the level of pain.

Frozen Shoulder Due to Systemic Disorders
A large population of patients who suffer from systemic diseases may be more likely to develop frozen shoulder. These systemic diseases include those who suffer from thyroid disorders such as hyperthyroid and hypothyroid, both overactive and underactive thyroid function. Other diseases include diabetes, cardiovascular disease, tuberculosis and Parkinson's Disease. Frozen shoulder treatment related to these diseases is paramount and the importance of early diagnosis followed by early manipulation and treatment are necessary, because more drastic treatments are often times not an option. Frozen Shoulder in Surgical Patients
Patients who have recently gone through shoulder surgery such as rotator cuff repair or repair to the labrum, one of the many tendons in the shoulder, may experience frozen shoulder. Patients who have had recent mastectomy or cardiovascular surgery are at risk, as well. The reason they are more susceptible to the disorder is lack of use of the shoulder. When the shoulder remains in the same position for a prolonged period of time, the joint stiffens and the pain sets in.

Once the surgical site heals sufficiently, physiotherapy and chiropractic manipulation are highly recommended to help return range of motion to the joint and reduce pain. A study published on August 11, 2014, in the scientific periodical the Journal of Upper Cervical Chiropractic Research, added further evidence showing that chiropractic care has a positive effect on blood pressure. Numerous prior studies showed that people with high blood pressure who received chiropractic care experienced a blood pressure reduction. One study even showed that people who had low blood pressure had their pressure return to normal.

In the opening of the study, the author points out, "According to the American Heart Association, hypertension and related cardiovascular diseases continue to be a leading cause of death in the United States effecting approximately 77.9 million adults." He continues by noting, "The American Heart Association reports high blood pressure was listed as the cause of death or contributed to over 348,000 American deaths in 2009."

In this study, twenty people were randomly divided into two groups. One group, the control group, received a simulated adjustment that sounded real but no movement of the spinal bones occurred. The second group received a real chiropractic adjustment. Follow-up data was taken from all subjects at nearly the same times so as to make the collection of results consistent.  The adjustments given to the subjects were for correction of detected Atlas (top bone in neck) subluxations.

To give both groups in the study the same experience, an instrument was used to render an adjustment to the group that was getting a real adjustment. In the control group, the instrument was set to make sound but not deliver a thrust, therefore simulating a real adjustment. After either the real adjustment or the simulated procedure, all patients were asked to lay on the table for one minute. Afterward, standard post-adjustment procedures were performed to verify that spinal changes occurred in the group that got the real adjustment, while no spinal changes occurred in those who got the simulated adjustment.

The results of the study showed that those who received the real adjustment had a significant reduction in both their systolic and diastolic blood pressure. The control group, who received the simulated adjustment, did not show any statistical change in blood pressure. The effects from the single real adjustment given in this study lasted approximately one month.

In the conclusion of the study, the author states, "In this investigation the correction of Atlas subluxation in the experimental group significantly decreased systolic and diastolic values for up to one month with only one therapeutic intervention." He concludes by suggesting, "The results of this study would suggest there would be a significant benefit in evaluating for and correcting any Atlas subluxation or malposition found in patients that suffer from ABP, (arterial blood pressure)."

The word  fibromyalgia comes from the Latin term for fibrous tissue (fibro) and the Greek ones for muscle (myo) and pain (algia). Fibromyalgia syndrome is chronic disorder which includes widespread muscle pain, fatigue, and multiple tender points that affects 3-6 million people in the United States. For reasons that are unclear, more than 90% of those who develop fibromyalgia are women. It is not currently known whether the predominance of women who suffer from fibromyalgia is a phenomenon of the socialization of women in the American culture or whether it is some combination of the female reproductive hormones and other genetic predispositions.

According to the American College of Rheumatology (ACR), fibromyalgia is defined as a history of pain in all four quadrants of the body lasting more than 3 months. Pain in all four quadrants means that you have pain in both your right and left sides, as well as above and below the waist. The ACR also described 18 characteristic tender points on the body that are associated with fibromyalgia. In order to be diagnosed with fibromyalgia, a person must have 11 or more tender points. In addition to pain and fatigue, people who have fibromyalgia may experience:

  • sleep disturbances 
  • morning stiffness 
  • headaches 
  • irritable bowel syndrome 
  • painful menstrual periods 
  • numbness or tingling of the extremities 
  • restless legs syndrome 
  • temperature sensitivity 
  • cognitive and memory problems (sometimes referred to as "fibro fog") 

Fibromyalgia is often confused with another condition called "myofascial pain syndrome" or "myofascitis." Both fibromyalgia and myofascitis can cause pain in all four quadrants of the body and tend to have similar tender point locations, but the two conditions are worlds apart. Myofascitis is an inflammatory condition due to overuse or injury to your muscles, whereas fibromyalgia is caused by a stress-induced change in metabolism and healing. Myofascitis tend to come on rather suddenly and is usually associated with a particular activity or injury, true fibromyalgia has a slow, insidious onset, usually beginning in early adulthood. It is very important to diagnose each of these correctly, for they require very different approaches to treatment. Unfortunately, fibromyalgia is a chronic condition, meaning it lasts a long time - possibly a lifetime. However. it won't cause damage to your joints, muscles, or internal organs. 

The Basics of Fibromyalgia

The latest research indicates that fibromyalgia is a stress-related condition that is a cousin in Systemic Lupus Erythematosis (often referred to as simply 'lupus') and Chronic Fatigue Syndrome. In all three of these conditions, there is the same predominantly female distribution, chronic fatigue, sleep disturbances, irritable bowel, as well as many other similarities. You can think about these three conditions as lying on a continuum with Fibromyalgia on one end, Lupus on the other and Chronic Fatigue Syndrome in the middle. All three of these conditions are caused by an abnormal stress response in the body, but with Lupus, the immune system is primarily affected, causing an autoimmune reaction that attacks your healthy tissues. On the other end of the spectrum is fibromyalgia, where metabolic abnormalities are primary. These metabolic changes are the result of a stress-induced decrease in blood flow to an area of the brain called the pituitary. This, in turn causes a decrease in a number of important hormones, such as the growth hormone releasing hormone (somatotropin) and the thyroid stimulating hormone. These hormonal changes lead to abnormal muscle healing, borderline or full-blown hypothyroid, as well as memory and cognitive changes.

One of the major physical abnormalities that occurs with fibromyalgia lies in the muscle itself, where there is a build up of a protein called "Ground Substance." Ground substance is normally found in muscle, bone and connective tissue all over the body and is responsible for making the tissues stronger and less susceptible to tearing. In a normal person, when a muscle is injured, the muscle tissue itself is able to regenerate and over time, completely heal itself. In a person with fibromyalgia, the muscle is unable to completely heal itself. Instead, an abnormally large amount of ground substance builds up in the injured area. It is the ground substance, coupled with local muscle spasm it creates that creates the muscle 'knots' associated with fibromyalgia.

A number of tests may be done to rule out other disorders and an examination can reveal whether a person has the characteristic tender areas on the back of the neck, shoulders, sternum, lower back, hips, shins, elbows, or knees. Unlike its cousin lupus, there are currently no diagnostic laboratory tests for fibromyalgia. Because there are no clinical tests for fibromyalgia, some doctors, unfortunately, conclude that a patient's pain is not real, or they may tell them that there is little they can do. But a combination of chiropractic, trigger point therapy, and lifestyle changes has proven to be very effective in decreasing the severity and duration of the physical pain and disability of fibromyalgia.

Fibromyalgia can be difficult to treat. Not all doctors are familiar with fibromyalgia and its treatment, so it is important to find a doctor who is. Fibromyalgia treatment often requires a team approach, utilizing chiropractic care, trigger point therapy, massage, dietary changes, as well as exercises and stretching.

Treating Fibromyalgia With Chiropractic

Chiropractic care is critical for those who suffer from fibromyalgia in order to keep the spine and muscles from losing too much movement. Because fibromyalgia causes the muscles to tighten up and lose some of their natural pliability, it results in a global loss of movement in the spine. The loss of movement in the spine results in a neurological reflex that causes the muscles to tighten further. This vicious cycle will continue and over time will lead to increased pain, increased muscle tightness, a loss of movement, more difficulty sleeping and the development of more and more trigger points.

The only option is to continually adjust the spine and keep it moving. It is not uncommon for those with fibromyalgia to be adjusted three to four times per month to keep everything mobile and relaxed. The biggest concern in treating people with fibromyalgia is that their muscles have a diminished healing ability. For this reason, chiropractic adjustments are usually modified slightly to be more gentle than normal. This helps to decrease the stress on all of the small supporting muscles of the spine, which can be easily injured. It is important when seeking chiropractic care, to make sure that the doctor is familiar with the muscular changes that occur with fibromyalgia so that they can adjust their treatment accordingly.

Treating Fibromyalgia with Trigger Point Therapy

The overwhelming characteristic of fibromyalgia is long-standing, body-wide pain with defined tender points, and frequently, trigger points. Trigger points are often confused with "tender points." They are not the same. A trigger point needs firm pressure to elicit pain, while tender points are painful with even very light pressure. Trigger points will refer pain to other areas of the body, whereas tender points will not. Unlike tender points, trigger points can occur in isolation and represent a source of radiating pain, even in the absence of direct pressure. As discussed earlier, trigger points are purely comprised of spasmed muscle fibers, whereas tender points are knots filled with ground substance. Those with fibromyalgia almost always have a combination of the two - trigger points and tender points - and can improve dramatically with light trigger point therapy.

Trigger point therapy for fibromyalgia is much like trigger point therapy for low back pain, neck pain or headaches. The points are the same. The difference is just intensity. Since the muscles in patients with fibromyalgia are easily injured and take longer to heal, it is necessary to use less pressure on their trigger points.

Treating Fibromyalgia with Cold Laser Therapy

Since poor healing of muscle tissue and chronic pain are characteristic traits of fibromyalgia, laser therapy is an important part of any treatment plan. Two of the major benefits of cold laser therapy is stimulation of tissue healing and decreased sensations of pain.

A 1997 study of 846 people with fibromyalgia reported in the Journal of Clinical Laser Medicine and Surgery demonstrated that two-thirds of the patients experienced improved pain and mobility with cold laser therapy. Another study published in Rheumatology International in 2002, showed that those who received laser therapy had a significant improvement in pain, fatigue and morning stiffness.

Self-Care for Fibromyalgia

Your day to day lifestyle choices have a tremendous impact on how much impact fibromyalgia will have on your life. The difference between those who take care of themselves and those who do not is tremendous. Those who make lifestyle changes to help their fibromyalgia suffer much less pain, are able to remain more active and have a much higher quality of life than those who do not. If you have fibromyalgia, here are some of the main things that you can do on a daily basis to help your body: 

Getting enough good sleep

-Getting enough sleep and the right kind of sleep can help ease the pain and fatigue of fibromyalgia, but is something that can be hard to get. Many people with fibromyalgia have problems such as pain, restless legs syndrome and brain-wave irregularities that interfere with restful sleep. Insomnia is very common. Although alcohol may help you to relax, it is not recommended before bed as it has been shown to interfere with restful sleep. Some of those with fibromyalgia have found 5-hydroxy tryptophan (5-HTP) very helpful, as well as the prescription anti-depressant amitriptyline. Typically, we don't recommend taking perscritpion drugs, but in this case, it is difficult to heal without enough sleep. 

Exercising

- Improved fitness through exercise is recommended. Studies have shown that fibromyalgia symptoms can be relieved by aerobic exercise. Though pain and fatigue may make exercise and daily activities difficult, it's crucial to be as physically active as possible. The best way to begin a fitness program is to start with low impact exercises, like walking and swimming. Starting slowly helps stretch and mobilize tight, sore muscles. High-impact aerobics and weight lifting could cause increased discomfort, so pay attention to your body. The more you can exercise, the better off you will be. 

Making changes at work

-Most people with fibromyalgia are able to continue working, but they may have to make big changes to do so. It may be necessary to reduce the number of hours at work, find a job that will allow you to have a flexible schedule, or switch to a less physically demanding job. Many people with fibromyalgia require specially designed office chairs, adjustable desks or other adaptations in order to continue working. If you face obstacles at work, such as an uncomfortable desk chair that leaves your back aching or difficulty lifting heavy boxes or files, your employer may make adaptations that will enable you to keep your job. 

Eating well

-Foods, just like anything else, have the ability to either stress your body or to help your body heal. Foods that tend to be stressful on the body include: dairy, eggs, wheat, corn, as well as anything with monosodium glutamate (MSG), nitrates or nitrites (as are found in processed foods). Several environmental toxins may also contribute to the overall physical stress on your body, therefore fish should be avoided as well. It is important that you eat as much clean, organically grown fresh foods as possible. Base your diet around whole foods such as: brown rice, legumes, oats, spelt, rice milk, soy, hormone-free chicken or turkey, roots, nuts and berries. 

Nutritional supplements

-There are dozens of nutritional products that claim to be 'the answer' for fibromyalgia. To date, none of them have proven to be of much long-term benefit for anyone. However, there are some people who have used magnesium malate with good results, some people who have used ginkgo biloba with good results and others with various herbals. The bottom line with nutritional supplements is that, to date, there is nothing that works for everyone. If you come across something that you would like to try, by all means do so, as long as you check it out with your chiropractor first to ensure that it won't interfere with any of your other treatment.
